The National Primary Health Care (NPHC) Conference 2016 is a platform for the whole primary health care community and professionals to come together in a supportive and collaborative environment to discuss pivotal issues for the future of primary health are in Australia and where delegates will learn from the experiences, opinion and perspectives of sectors leaders and peers.
The NPHC Conference 2016 theme will be supported by complementary streams focusing on Community, Clinical and Governance. Each stream will incorporate a strong focus on prevention, consumer engagement, building capacities for improving patient outcomes with an emphasis on mental health, Aboriginal and Torres Strait Islander health, population health, health workforce, diversity in practice, chronic disease, eHealth and aged care.
Stream 1: Community
Focusing on community and consumer prospective, this steam will look at ways primary health care services are aligned to local experiences, requirements and expectations of the community and consumers. This is an opportunity to share experiences and address issues unique to health including access to services and service gaps. It will also be an opportunity to discuss and share experiences in communicating and engaging with local communities and consumers.
Stream 2: Clinical
Coordination of local health care systems are vital for patients to reduce preventable hospital admissions. Primary Health Networks (PHNs) are required to work in partnership with Local Hospital Networks (LHNs) to develop local strategies to improve operation of the health care system. This stream will focus on coordinating resources to improve health outcomes, pathways between hospital and general practice, streamlining patient care and cost-effective and innovative was to improve rural and remote access to health care.
Stream 3: Governance
PHNs are required to operate effectively and efficiently to deliver against nationally and locally. This stream will focus on performance, outcomes, financial, risk, legal and business management systems in primary health care.
It will also focus on governance of PHNs and coordinate networking and resource sharing particularly in relation to organisational structure, commissioning, purchasing and providing services.
Conference objectives
