We provide this support through recovery, education and advocacy programs and provide a range of innovative rehabilitation programs. We currently employ over 450 staff and work across multiple sites within Victoria, Tasmania, New South Wales, Queensland and the ACT.

About this opportunity

Partners in Recovery is a community mental health service that specifically works with people with severe and persistent mental illness with complex needs, and with their carers and families.

As a Support Facilitator, you will support the participant to navigate the service system and access appropriate services at the right time and in the right place, and support service providers to work together to coordinate care. You will strengthen partnerships, improve referral pathways and promote a community based recovery model.

There are 3 x Part Time fixed term (0.8 time fraction) opportunities until 30 June 2017 with potential for extension. You will be based in Frankston VIC, however you may need to travel on occasion.

You will possess the following:

  • tertiary qualification(s) in social science or similar discipline or relevant experience,
  • demonstrated experience working in a recovery orientated mental health environment,
  • current and valid Driver’s Licence,
  • satisfactory Police Records History, and
  • current Working with Children Check.
