Emerge Case Manager & Housing Specialist

About the role
The Emerge Pre and Post Release case management program will provide 6 weeks pre-release and up to 6 months post release case management support to men exiting Ravenhall CC. The Housing specialist in this team will carry a mixed case-load of longer term case-management, and short term appointment based housing assessment and referrals. The role will also participate in delivering housing information sessions within the Centre. The position will be located at both Ravenhall Correctional Centre and the Community Re-integration Centre located in Richmond.

Duties will include but not be limited to:
• Delivering a high quality pre and post release case management service to men exiting Ravenhall CC.
• Providing case management support in a holistic and individualised manner.
• Undertaking reviews of assessments and complete checklists for eligible participants.
• Building positive relationships with program participants to facilitate the implementation of their reintegration plan.
• Working cooperatively and collaboratively with GEO reintegration officers and other employees at Ravenhall CC.
• Liaising and working co-operatively with other programs, community agencies, and employment, education and training services.
• Advocating for clients to community agencies, employment, education and training services, real estate agencies, court, etc.
• Liaising with local area service networks and state-wide programs for the provision of housing and resources.
