About the role
THE POSITION:
Reporting to the Client Services Manager, you will be responsible for the assessing, planning and coordination of services across programs (CHSP, HCP and DVA)
ESSENTIAL CRITERIA:
• Diploma level qualification in Aged or Community Care or related field, and/or experience in similar capacity
• Experience in Client Care Coordination
• Understanding of Case Management or how to reference Case Management for clients or families
• Ability to work with a high level of independence and autonomy
• Ability to support and supervise Bilingual Support Staff
• Good administrative and computer skills to maintain client files,
• Effective communication, negotiation, organisational, time management and customer service skills
• Ability to work in a multi-disciplinary environment
• Ability to speak a second language
• Demonstrated ability to be details orientated
• Willingness to undergo a National Police Check
• A current driver’s license and own vehicle
DESIRABLE CRITERIA:
• Applied knowledge of enabling consumers to achieve independence
• Experience in writing and submitting reports
