We’re offering a great opportunity for an experienced allied health professional to deliver care coordination to clients presenting with a range of health needs relating to HIV, mental health, suicide risk, substance support as well as ageing, domestic and family violence and housing. This position is suited to someone with experience working in multidisciplinary teams within a chronic disease self-management model.
