Healthy Ageing Project Manager

Job Description
An exciting opportunity has arisen at the North East PCP to support partner organisations to collaborate on initiatives aimed at promoting healthy ageing. The Healthy Ageing Project Manager leads our partnership in a collective approach to supporting older people to be active and healthy. We are seeking an experienced and qualified professional with a successful track record of working with numerous partners to achieve better health outcomes.

Banyule Community Health is the auspice organisation for the North East PCP and as such employs our staff.

The key responsibilities for this position include:

• Facilitate and coordinate integrated, collaborative activity to promote healthy ageing across the North East PCP catchment
• Coordinate and support the development, implementation and evaluation of a catchment-wide healthy ageing plan
• Gather, interpret and translate data, evidence and knowledge to support a collective impact approach to healthy ageing
• Source external funding opportunities to support initiatives for priority areas and facilitate the preparation of partnership based submissions

The key skills required for this role include:

• Relevant tertiary qualifications preferably at post-graduate level
• Demonstrated success in facilitating the development of multiagency networks in a community or health services environment
• Demonstrated success in developing, implementing and evaluating social justice or public health initiatives to achieve improved population health outcomes
• Knowledge and in-depth understanding of the ageing sector, the social determinants of health, health promotion principles and practices, health promotion planning, and evidence based approaches to prevention
