An exciting opportunity currently exists within the Community Care Team at Mosman Council. We are seeking an enthusiastic person who has strong interpersonal and communication skills to act as Care Programs Officer between clients and key services.

Mosman Community Care provides a range of person centred services to older Mosman residents and those who are frail aged, people with a disability and their carers. The Care Programs Officer will be responsible for the referral, intake, assessment and follow-up of all clients referred to Community Care Services, mainly for Social Support, Food and Transport Services, and will work closely with other services to ensure a smooth transition for clients. This involves managing all referrals from My Aged Care Portal and developing a plan for the quality and smooth transition of clients into appropriate service/s. The successful applicant will also be responsible for coordinating social support groups, will assist with the efficient and effective operation of Mosman Community Care and will be a team player.

This is a part time position Tuesday to Friday 8.30am to 5pm for 30 hours per week. Twice a month you will be required to work on a Saturday to coordinate the Social Day Program for older adults. On those occasions your work week will be Wednesday to Saturday.

Skills in dealing with all client target groups, a driver’s licence, service promotion and high levels of computer literacy are required.
