About the role

  • Advance your career and support integration across the primary health care system
  • Salary $75K – $85K + Superannuation, with access to salary packaging
  • Fixed term appointment to June 30 2020

 

Reporting to the Manager, Integration, the role of Program Officer, Integration is to support service and system improvements in palliative care across and between primary, secondary, tertiary,
residential and community health services. This includes a focus on reducing avoidable hospitalisations and improving shared care and communication across these intersecting care settings. Along with your existing knowledge of and experience working within the Victorian Public Health system, you will apply your program and  project management skills to deliver improved health outcomes for our community.

To do this you will utilise our commissioning framework, and integrate data, evidence and evaluations to drive service improvements. Please review the Position Description (available from our recruitment page) for further information.

What you will bring to the role:

  • Demonstrated knowledge and understanding of the Victorian health service system and contemporary health policy issues across the acute and primary care interface;
  • Proven program or project management expertise;
  • Sound stakeholder engagement and relationship building skills
  • Proven aptitude for technical systems – familiarity with MS Office 365 and SharePoint will be an advantage
  • Ability to apply a collaborative approach to your work
