Embedded into the Initial Assessment & Planning team, you will deliver targeted case management housing and support services to families and children who are homeless or at risk of homelessness.

In this challenging, yet fulfilling role, you will be responsible for providing outreach services that assess, monitor and evaluate the housing and support needs of a case-load of families and children; assisting them to move from inappropriate, unsafe or unaffordable rooming house accommodation into short-term accommodation, or select private rental, and with a view to assisting them into long-term sustainable housing.

You will also assist in linking families with appropriate long-term support services and assist families and children to positively engage in their communities of choice.

With an understanding of the prevalence of domestic violence, its’ impact on families and children, and also of the child protection system, we are seeking an individual who is committed to making difference to vulnerable families.
