About the role
- 0.8 – 1.0 EFT + ADO (optional)
- Fixed term until 30th June 2019
- Based in Sunshine
Delivering the next generation of community mental health care excellence, cohealth Community Mental Health service comprises highly experienced teams, who work with clients who have the most difficulty accessing services. Our focus is on the most disadvantaged, underpinned by our understanding of the social determinants of health, informs our efforts to enable service delivery to the people who need it most.
About the role
Operating in a dynamic space, this role focuses on supporting clients in their mental health recovery and improving wellbeing. Successful applicant will also be responsible for successfully transitioning clients, into the National Disability Insurance Scheme as well as providing expertise and support to the local team, through this process.

- A sound understanding of the key issues affecting people with multiple and complex needs;
- Experienced in comprehensive assessment and care planning;
- Good understanding of the NDIS;
- The ability to work well within a team, being both accountable for your own practice as well as driving accountability in other team members also; and
- A lived experience of mental illness as a consumer, family member or carer will be highly regarded.
