Case Worker (Permanent full time) – Coffs Harbour

We are looking for a passionate permanent full time Case Worker (within our Supporting and Securing Tenancies program) to join our team in Coffs Harbour. In this position, you will report to the Team Leader and provide early intervention and support to people who are at imminent risk of becoming homeless, currently experiencing homelessness and/or living in unsustainable and unsafe accommodation.

You will collaborate with customers to develop and implement person centred plans, build individual and family capacity considering the needs of children, enhance living skills, resilience and community engagement, to assist with finding suitable long term accommodation.

The day to day

• Provide information, advice and advocacy to customer utilising a person-centred approach

• Regularly review content of the Customer Management Systems (ie, Carelink or CIMS) to ensure customer documentation is accurate and current

• Develop and review case plan to reflect customer’s need and preference

• Understand and apply NDIS and other relevant services guidelines, framework, legislation and standards

What you need to have:

As well as being a person centred super star you’ll have:

• Passion, Respect & Integrity
• Technical literacy with proven ability to use Microsoft Office and systems for reporting
• A relevant tertiary qualification and/or equivalent knowledge and experience
• Valid Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans
• Must be fit to undertake the physical requirements of customer support, including the capacity to lift 10 kilograms unassisted
• Willingness to obtain an Australian Federal Police Check and WWCC that meet New Horizons employment criteria prior to commencing with us
