About the organisation
Glady and Co is an innovative new organisation that supports people with mental health difficulties by providing flexible, out of the box NDIS services. We specialise in lived experience support and creating services that flexibly evolve to meet the needs of our clients throughout their lives.
Glady and Co has recently opened a Recovery Hub in Mooroolbark, we offer a group program and drop-in facilities through NDIS funding. All staff members have the opportunity to facilitate groups and participate in social outings. The Glady and Co Hub aims to be a community within itself that focuses on increasing participants skills and abilities to actively participate in the greater community.

About the role
- 5 x Transitional Support Connection Officers
- Mooroolbark, Victoria and option of working from home
Glady and Co are a mental health service, located in the Eastern Suburbs, that provides outreach mental health support, support coordination, allied health services and a group/drop-in centre. Glady and Co specialises in lived experience support work and the ability to utilise life and recovery experience is held in high regard, Glady and Co are a supportive and family friendly organisation that provides ongoing flexibility for it’s employees.
TRANSITIONAL SUPPORT CONNECTION
The role of Transitional Support Connection Officer is a varied position that involves:
- Assisting participants to access support coordination
- Supporting participants while waiting for support coordination
- Connecting participants to mainstream services
- Assisting participants to self-advocate for changes in their NDIS plans when needed
- Providing Mental Health NDIS participants interim supports – people currently receiving support outside of NDIS are immediately exited from services once they are given an NDIS package, the Transitional Support Connection Team will ensure that these participants do not fall through the gap between mainstream and NDIS services.
Each NDIS participant will have different needs, this role varies with each person and Glady and Co aim to provide a unique experience for all participants.
About You:
You will have experience in providing mental health support within community services, have an excellent understanding of the varied ways mental illness can effect aspects of the participants life and family dynamic, be focused on quality service delivery for customers that have an NDIS plan. You will be efficient, organised, able to balance the demands of client interface and administration tasks. You will have excellent conflict resolution, case management and listening skills.
