About the role
- Part-time hours – 30 hours per week (hours/days of work flexible)
Who we’re looking for
The position requires a highly motivated, well organised person, who can effectively engage individuals and community stakeholders, build positive and sustainable relationships, and demonstrate a person-centred approach.
What you’ll be doing
Home Care Package Case Managers are expected to work collaboratively with the customer and their support network to monitor and review the individual plan, ensuring it remains responsive to changing needs and circumstances and within an agreed budget plan.
The successful applicant will need to draw upon and build on relationships with colleagues, local government, service networks and a range of community agencies.
