We are seeking an enthusiastic Case Manager to join our Melbourne Street to Home team (MS2H) at our Launch Housing St Kilda site. MS2H is a partnership program between Launch Housing, The Salvation Army Adult Services and Crisis Services and Bolton Clarke (formerly RDNS) – Homeless Persons Program. The program provides intensive case management support to clients deemed most vulnerable; those who have been homeless the longest and those who have serious physical health conditions, mental health issues and substance misuse issues. MS2H provides coordinated housing, support services, and health intervention. It is a housing first program which aims to provide a rapid housing response and intensive support pre and post housing.

The Case Manager role involves assertive outreach to rough sleepers, intensive case management and relies heavily on effective collaboration with clients and external services. The successful candidate will have excellent interpersonal skills, take accountability for their work outcomes and be able to form and maintain strong relationships, which are all essential ingredients to working collaboratively with clients and other organisations to achieve common goals. The successful candidate will also have outstanding time management skills with the ability to self-direct and work independently when needed. Experience working with clients sleeping rough as well as vulnerable clients, who have multiple and complex needs will be highly regarded.
