We have full time roles on maximum term contracts till 30 June 2020 with the potential for extension.
Rotating roster as follows;
- Monday to Friday; 8am – 4:30pm, 12.30pm to 9pm
- Saturday and Sunday; 8am – 4.30pm
Day to Day duties
- Collaborating with customers to develop individualised support plans that incorporate their identified wellbeing needs and goals
- Providing support to customers to improve their independence by assisting with living skills, transport, social activities and medication as well as other support as required
- Supporting customers to explore their practical and complex needs
- Keeping customer records up to date on a daily basis using on line systems
- Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
- Seek out innovative ways to enhance the customer journey
What you need to have:
- Relevant qualifications and/or industry experience
- Passion, Respect & Integrity
- Self-motivated with the ability to work autonomously and in a team environment
- Ability and confidence to communicate and work with other staff outside of the immediate team
- High level of accuracy and attention to detail
- Ability to multi-task and manage competing priorities according to agreed
timeframes and quality standards - Understanding of Recovery Orientated Practice
- NSW Driver Licence or Driver Licence valid for use in NSW
