An exciting opportunity exists for an enthusiastic and experienced Registered Nurse to take our Home Care Package Program to the next level.

The Home Care Coordinator works closely with the Home Care Package Program Care Advisor and Service Coordinator, providing leadership and operational management of the Home Care Packages (HCP) team whilst actively contributing to the ongoing development and expansion of the Program.  

The Home Care Packages Program provides Consumer Directed Care through the provision of care, clinical support and monitoring, advocacy and service co-ordination to older people who want to remain living in their own home. 

The focus of the Home Care Program is to support our consumers and their families with access to a range of ongoing personal services, support services and clinical care that re engage consumers to manage their day-to-day activities.  

The Home Care Package Program forms an integral part of the Australian Government’s continuum of care for older people in Australia. 

To be suitable for the role you will possess the following:

  • A Degree in Nursing
  • Previous leadership experience in managing Home Care Packages Program
  • Relevant experience and capacity to provide high level Leadership, Support and

       supervision to staff

  • Demonstrated experience in program development and evaluation
  • Demonstrated high level interpersonal, facilitation and consultation skills and the ability

        to effectively communicate with, senior management, internal and external stakeholders

        and staff.

  • Sound understanding of clinical governance
  • Demonstrated experience in budgeting and accountability reporting
