Over a three-year journey, we will be delivering cutting-edge interventions with proven outcomes in stable housing, social inclusion and economic participation.

About the role

Your new role

The Intensive Case Manager is designed to foster the independence of our participants and support them to achieve the life they want to live. We are looking for dynamic and motivated team players who have:

  • Relevant degree or above qualifications
  • Experience working with participants with complex needs
  • A strength-based approach
  • Resilience, flexibility and adaptability
  • Creative outcome-focused attitude

Your day-to-day

While every day is different, a typical day might see you on outreach, visiting one of your many participants and linking them to appropriate services. You may then return to the office for a check-in or group reflective practice with your colleagues.

For the rest of the day, you will likely be:

  • Utilising our Case Management Framework to progress participant goals and ensure strong service delivery outcomes
  • Working with participants to develop support plans that are holistic and contain clear, specific strategies for sustainable change
  • Providing assertive and planned outreach to participants in their preferred environment
  • Developing safety assessments with risk management strategies
  • Service coordination with other agencies or partners

We’re looking for someone

  • With relevant degree or above qualifications

You can look forward to

  • Working part time (19 hours per week)
  • Job security of maximum term contract until 30 June 2021
  • Annual Salary Range of $69,305 – $74,776 pro-rata plus 17.5% annual leave loading
  • Salary packaging your expenses up to $15,990 pre-tax
  • Structurally supportive work environment: regular line manager direct supervision, 1-on-1 clinical supervision, in-house training dept, group reflective practice, team days, access to an employee assistance program
  • Opportunity to purchase additional leave
  • Parental leave; 10 weeks primary carer, 3 weeks secondary carer
