Reporting to the Manager – Home Care Packages, the Case Manager is accountable for overall performance and service delivery through managing the care and services of our customers within a Consumer Direct Care framework, for the aged living in a home care setting.

About You
We are seeking an energetic and self-directed Case Manager who is looking for a role where you will have direct customer contact, can provide quality care in your local community and be part of a growing team in the HCP space.

You are self-motivated, flexible and have excellent time management skills. You are a high energy individual who loves the challenge of driving change in an evolving aged care landscape whilst ensuring that we provide the best levels of care to our customers. You are a strong communicator, pragmatic and demonstrate strong interpersonal skills. You are able to balance customer choice, with the needs of the business and a highly compliance focused industry, this is critical role to the future path of our Home Care Business.

 

Key Requirements (Selection Criteria)
• Degree or Diploma in a Community, Health or Management discipline
• Advanced competence in management information systems and software packages such as Microsoft Office and Carelink +
• Minimum 3 years experience in a community services case management role
• Demonstrated understanding of Consumer Direct Care obligations for Case Management
• Proven ability to negotiate, manage and operate within a budget
• Current Victorian Driver’s Licence and own reliable vehicle with comprehensive insurance
• Must be flexible and self-motivated with excellent communication skills
• Current valid Police Check and a Valid Australian Working Visa are required prior to employment
• Home internet required
