Every day of the year, Sacred Heart Mission assist hundreds of people who are experiencing homelessness or living in poverty to find shelter, food, care and support. Your new role

Our Assistance with Care (ACH) Worker works with clients over the age of 50, supporting them to achieve their goals through planned support and linkage work. You will work with clients to complete aged care assessments, sustain housing tenancies, referrals to housing, health and legal. This will be done in accordance with the organisation’s values, practice principles, legislative and funding requirements, as well as building relationships with community agencies, local services and other Mission services.

Two days a week you will be working in our engagement hub (300 people attend daily) where you are an initial point of contact for clients accessing services. This provides opportunities for you to build rapport with eligible individuals.

This is a maximum term contract to 31st June 2022.

Your day-to-day

Three days a week you will be providing direct support and advocacy for clients in the ACH program. This will include outreach, assisting clients to appointments and administration duties. You will receive referrals from our engagement hub, our Woman’s House, Caulfield Hospital and Launch Housing.

Two days a week you will be based at our 87 Grey Street Engagement Hub where we serve breakfast and lunch to approximately 300 clients every day of the year. You’ll be making contact with new and familiar clients, providing support, information, assessment, crisis intervention and advocacy. You’ll be working closely with colleagues and workers from other services.

We’re looking for

  • An energetic, resilient person who can work with flexibility and calm in a dynamic environment
  • Someone with experience working with complex clients
  • Someone who is team-focused with strong communication skills
  • Flexibility and the ability to respond to an ever-changing environment
  • Someone who is able to respond effectively to challenging behaviours
  • Someone with experience in administration and great time management
  • Someone with an appropriate tertiary degree or associate diploma relevant to the delivery of community services work e.g. social work, community development, nursing or equivalent, or relevant diploma with substantial experience.
