The Role
Transitional Support Program (TSM) is a federally funded program carried out by a team of Support Facilitators who assists clients, their families and carers to better navigate the complex mental health and allied service & support systems, and to assess, review and coordinate the full range of services required to assist clients to live a “contributing life”.
About you
The successful candidate will have previous experience in the social welfare sector, working with clients with complex mental health needs and social policy/client support frameworks and a thorough understanding of the Mental Health service system including clinical and non-clinical services within the region. Working in a dynamic multi-disciplinary team, this challenging position requires someone with strong communication skills, well-developed organisational and time management skills.
