We are seeking a committed and experienced support worker to support people with multiple and complex needs that require short- or long-term support with obtaining and retaining appropriate housing and relevant supports.
Haven; Home, Safe is a for-purpose organisation which connects people to housing options and support services to enable them to find and keep a place to call home.
Community Outreach Workers work within the community and with various agencies to identify and support people whose needs are often overlooked by mainstream services. This includes those who are experiencing multiple and complex health problems and who are homeless, living in inappropriate or unsafe housing or at risk of becoming homeless.
Community Outreach Workers will operate within a dynamic and empowering framework of assertive outreach, service linkage and support.
The Community Connection Program helps Victorians with multiple or complex needs who are homeless or live-in low-cost accommodation. Often these people are isolated and not well connected with health, housing or community services. They easily ‘slip through the gaps’. The Housing Support for the Aged Program (HSA) assists people aged 50 and over with multiple or complex needs who require ongoing support to live independently in public or low-cost housing.
