Position: Case Manager – Home Based Care

Anchor has a long history of providing services for children and young people unable to live with their family, for young people who are at risk and for individuals and families who are experiencing housing stress or homelessness.
Who we are looking for:
If you are a degree qualified (preferably Social Work, Psychology and/or related behavioural sciences) and an experienced Case Manager in Home Based Care with a proven track record and a passion for making a difference, you may be exactly the person we are looking for!
As part of our team, you will support Anchor by providing high level case management skills to ensure effective placements in foster care. You will offer a broad range of abilities to accomplish exceptional service delivery and development
What you will be doing:
Utilising your exceptional skills and experience, you will be managing case work, supervision and support for carers, children, young people and their families in accordance with the Looking After Children Framework.
You will have a real talent for communication and problem solving as you participate in consultation processes regarding service improvement and development to elevate our Out of Home Care Program.
You will be collaborative, reliable, hardworking and have the capacity to exercise professional judgement to ensure excellent outcomes for our clients and the team.
Who you will be working with:
The Foster Care team is one of the two programs that make up the larger Out of Home Care Team, the other being the Kinship Care team. You will be part of a committed Foster Care team led by a hands-on Team Leader, ensuring that you are be supported to do your best work.
