About the role
- Part-time 0.5 EFT (negotiable), Fixed term role until 30th April 2024
- Role-based in Fitzroy or Braybrook but may require working across sites occasionally
- $34.82 hourly + Super + generous salary packaging
About the role:
The role of Social Emotional Wellbeing Worker – Integrated Team Care sits within the cohealth Yakeen Tharn (Aboriginal and Torres Strait Islander Health Team) Services program, which aims to support the chronic health conditions of people who live in the municipalities of Maribyrnong, Melbourne, Moreland, Brimbank, Moonee Valley, Hume, Darebin and Yarra.
A key focus of the role will be upon establishing and delivering individual and culturally appropriate group-based programs which support and improve Social and Emotional Wellbeing outcomes.

About you:
- Experience working with Aboriginal and Torres Strait Islander communities and people in urban settings.
- Being culturally respectful, flexible, using initiative, professionalism, decisiveness, integrity, attention to detail, service orientation and outcome orientated.
- Well-developed computer skills including Word, Internet and other Office applications, willingness to learn current data programs such as TRAK Care, PHN computer systems
- Willingness to participate in appropriate training and commitment to ongoing education
- Demonstrated knowledge of the prevalence, impact and effective management of chronic health conditions in the Aboriginal or Torres Strait Islander population.
