An opportunity exists for an experienced Care and Recovery Coordinator to work within the SURe Program.

The Position:
The SURe consortium is comprised of Anglicare Victoria, EACH and YSAS and is a leading provider in the delivery of a broad range of community based Alcohol and other Drug treatment across the Inner East, Outer East and the South East.
The Care and Recovery Coordinator will hold a combination of responsibilities with regards to the Care and Recovery Coordination role. The position objectives and key responsibilities are as follows:
• Delivering care coordination to highest need/highest risk clients who present with complexity
• Provide comprehensive assessments including mental health screens.
• Develop Individual Treatment Plans for program participants.
• Assist clients to work towards harm minimisation strategies.
• Assist participants to utilise support networks.
• Enhance ability of individuals, family members and significant others to respond to drug and alcohol issues.
• Organise and facilitate care planning meetings with external agencies and clients (where appropriate).
• Assist in the enhancement of life management of the individual client/family.
• To maintain accurate and timely client records, alongside of relevant data collection relevant to service provision

To Be Successful in this Position You Will Need:
• Tertiary qualifications in a relevant discipline such as Social or Welfare Work, including minimum AOD qualifications including dual diagnosis competencies.
• Knowledge, experience and practice in models of contemporary approaches in the alcohol and drug field.
• Demonstrated ability to work with mandated clients
• Highly developed communication skills (written and oral) and conflict resolution skills
• A demonstrated ability to be receptive and flexible to new ideas and the capacity to adapt to changing work demands and circumstances
• A demonstrated ability to be a good team player and co-operate and support others in the pursuit of team and management goals.
• Good computer, organisational and time management skills and the ability to be self-directed, whilst working within a team.
