About the role
We have recently expanded our community services into Home Care and require an experienced and energetic individual to launch, promote and grow our home care services client base.
Reporting to the Community Services Manager you will be responsible for implementing and promoting the Home Care Program in accordance with the programs growth plan as well as the management and development of the program including growing the team to ensure delivery of Consumer Directed Care (CDC) packages and fee for service clients.
You will have a through understanding of consumer directed care, relevant legislation and government guidelines combined with strong networking, promotion and project management skills. Additionally you will be driven, flexible and have a ‘can do’ and collaborative attitude.
The Home Care Program Coordinator is responsible for:
•Developing, coordinating and implementing a marketing and promotional plan for the home care program in conjunction with the Community Services Manager
•Developing networks and maintaining relationships with key stakeholders to raise the program profile
•Assessing and developing care plans in consultation with clients, their family, carers and health professionals to meet client goals and needs.
•Overseeing all consumer directed care practices and monitor client needs and packages
•Recruit, manage and develop staff to ensure the ‘right skills are in the right place at the right time’
•Financial management of the program including coordination of Medicare reimbursement claims and client statements
•Undertake business and financial reporting
•Services are provided in accordance with SAAS policy and procedure, legislation, government guidelines and where required develop or update policy and procedure.
•Continuously improve the program by reviewing care delivery outcomes, completing risk, compliance and quality reporting requirements
To be successful in this role you will have:
•An undergraduate qualification in Nursing, Allied Health or Social Work or similar
•Postgraduate qualifications in Case Management, Community Care, Aged Care/Gerontology, Dementia or Chronic Disease Management (highly desirable)
•At least 3 years’ experience and knowledge of care coordination and case management within Community Aged Care
•Experience in management of a team of administration and service delivery staff, preferably within the Community Aged Care sector, including staff supervision and ongoing performance management
•Proven contemporary knowledge of consumer directed care and solid understanding of service delivery, regulatory and funding requirements to meet high/complex care needs of clients
•Project implementation experience including promotion of new initiatives including change management and meeting deadlines
•Highly developed communication and networking skills to initiate and drive program development
•Financial management, analysis and reporting skills required to effectively manage the home care packages program within set timeframes and budgets
•Demonstrated problem solving, negotiating and conflict resolution skills
•Effective management of quality/continuous improvement, risk and compliance processes
•Highly developed writing, interpersonal and facilitation skills
•Demonstrated high level flexibility and a team oriented approach
•Ability to influence and negotiate with diverse groups to promote consumer engagement
•Computer proficiency (including MS Office, Power Point, Excel and industry specific software applications)
