Community Options is an ACT community based non-profit organisation providing services to older people, people with disabilities and their families. We specialise in providing coordination and intensive case management services to people with complex needs by providing a range of in-home and community based support services across multiple systems and government funded programs such as the National Disability Insurance Scheme (NDIS).

Our Mission  Community Options is a not-for-profit community-based organisation that provides care and support to older people, people with disabilities, their families and friends. Our mission is to provide support services to our clients that will enable them to achieve a greater degree of independence and continue living quality lives in the community.

Our Vision Community Options' vision is for older people, people with disabilities and their families and friends to live good and full lives. We believe that this can be achieved through the person continuing to be a contributing and valued member of their community. To do this we aim to support people to maintain relationships, roles and connections to enhance the opportunities for people to enjoy life.

About the role

We are looking for a passionate and proactive individual with experience in complex case management, who has a thorough understanding of government and community programs. Your energy and exceptional communication skills will see you developing positive and cooperative relationships with our clients, service providers and government representatives.

Important Notes

The position is a 12 month contract with the possibility of extension. Three months probationary period will apply.

A salary package will be negotiated with the successful candidate, based on level of skill and experience.

Applicants must hold a Working with Vulnerable People (WWVP) card or be able to successfully register for a Working with Vulnerable People (WWVP) card.
