Uniting Wyndham has an exciting opportunity for a passionate Housing Support Case Manager within our program for Young people, Single adults, and Families. Uniting is currently undergoing an exciting transformational journey and we welcome you to join us. Within this role you will provide, housing support to the most vulnerable members of our community. Case manage low to complex families, whilst undertaking holistic assessments to determine relevant support.
About you
You will be a passionate, dedicated Case Manager with a drive to support clients and families to achieve the best possible outcomes. You will bring a wealth of direct service case management and prior exposure to the housing system platform “SHIP” would be advantageous. You will also provide information and support to families and single adults who are homeless or at risk of homelessness.
