Your impact to the community:

Launch Housing provides a range of flexible, responsive and professional services for people experiencing homelessness or who are at risk of being homeless across Melbourne. We actively engage clients to assist them to gain and maintain sustainable housing and other supports. The program works in partnership with other services to keep people safe, build rapport, and engage them with the broader service system and their community.

 

As a Case Manager, you will report to the Coordinator and will:

  • Engage with clients to determine eligibility for the organisation’s services and referral potential clients to other more relevant services if required
  • Develop case plans in conjunction with clients and implement strategies that enable clients to achieve their goals by building on their strengths and focusing on solutions
  • Assist clients to develop longer term housing options, both within and/or independently of Launch Housing’s supported accommodation program
  • Engage with clients to improve their quality of life and social inclusion in the community
  • Ensure clients maintain safe, appropriate and sustainable accommodation
  • Work in an assertive outreach capacity with clients assisting them to provide practical support and make appropriate referrals

 

To be successful in this role, you will have:

  • Experience in a case management role
  • Tertiary qualification in human services, social work, nursing, psychology, occupational therapy, or other relevant allied health disciplines
  • Understanding of housing and homelessness policy, context and systems
  • Well-developed IT and administration skills.
  • A demonstrated capacity to work flexibly, and have the ability to manage competing demands and the setting of boundaries and limits.
  • Ability to work collaboratively with other organisations in formal and informal partnerships to achieve client outcomes.
  • Valid Victorian Driver’s Licence
