The Partnership
COORDINARE, the South Eastern NSW Primary Health Network (PHN), in partnership with the Illawarra Shoalhaven Local Health District (ISLHD) and Southern NSW Local Health District (SNSWLHD) are planning an exciting collaborative commissioning program to develop and implement care pathways, aimed at improving health outcomes for people living with chronic conditions in their shared region.
This partnership and collaboration is built on a strong foundation of success and aims to ensure people have access to safe, appropriate and quality health services that deliver strong health outcomes in a community setting covering an area of 50,000+ square kilometres across twelve local government areas. Previous success includes implementing the first tri-partite Regional Mental Health and Suicide Prevention Plan and launching a cross-district virtual care platform that provided remote in-home monitoring as part of the Covid-19 response.
The Role
Reporting to the South Eastern NSW Patient‐Centred Co‐commissioning Group (PCCG), and organisationally to the COORDINARE CEO, this role will lead the development and implementation of this exciting collaborative commissioning program.
With tri-partite Chief Executive sponsorship, supported through joint accountability and governance mechanisms, the Program Director – Collaborative Commissioning will be well supported to guide the joint development and implementation stages of the collaborative commissioning program.
Key responsibilities include:
- Use Collaborative Commissioning as a funding mechanism to enter into outcomes-based contracts with service providers
- Lead the negotiation of contracts and the development of improved service models benefitting those living with chronic disease, reducing the need for these people to seek hospital care
- Co-design with consumers and stakeholders to understand and improve approaches to care
Skills Required
You will be passionate about contributing to improved health outcomes and have extensive senior management experience in health systems, program management and evaluation including complex program delivery. You are also skilled and enjoy engaging and leading communities and stakeholders in co-design processes to inform planning and decision making. You will also have:
- A deep understanding of the Australian health system, how healthcare is delivered, funded and managed in different settings, including health service commissioning
- Relevant tertiary qualification in health, management, business administration or similar
- Track record of effectively managing change and delivering results
- Sophisticated, tailored and nuanced written and oral communication skills
- A passion for achieving measurable outcomes and operate in an analytical environment interpreting and utilising data to draw insight and support arguments at a strategic level
