About the organisation
At EACH, our vision is for a healthy and inclusive community.
Founded on the vision that everyone is entitled to good health, we’re one of the nation’s leading health and community service providers, offering a broad range of services for over 40 years. Our services include Community Health, Mental Health, NDIS, Counselling, Support for Older Australians and Family Services.
About the role
Are you passionate about improving the lives of people with disabilities? Do you thrive in teams that encourage you to use your customer-centric approach to enable others to build confidence in their capabilities?
If you’re degree qualified in a Health or Human Service related field, with experience in a similar role, enjoy variety in your day, and work collaboratively with other professionals, you might be the next member of our expanding team!
As part of our Support Coordination team based in Narre Warren, EACH is looking for experienced Support Coordinators to join us in this rewarding high-growth area in a full-time ongoing capacity.
In addition to your ability to apply customer experience principles to your work, you will have an excellent ability to foster and nurture customer relationships, a desire to help people obtain their personal wellbeing and life goals through an empowerment-based approach, and experience in clinical, educational or community-based disability, mental health or early childhood intervention services or related fields.
What you’ll be doing
Your day will see you helping customers develop the skills and confidence needed to manage their NDIS plan. Initially, your role will be to help customers get started with their NDIS plan; supporting the implementation of all supports in their NDIS plan. You will act as the key liaison point for all related services selected by the individual. Over time, however, you will play an integral role in helping each participant build their own capability to take ownership of the Support Coordination function over time.
Services will be provided through outreach, telehealth, office-based and working from home.
Who you’ll be working with
Each regional team is comprised of one or two Team Leaders, a growing team of Support Coordinators and supported by the National NDIS Manager. You will primarily work collaboratively with the participant and local service providers.
To avoid a conflict of interest, you will work independently of other EACH teams that provide services under NDIS.
