Aged Care & Disability Support Coordinator

This is an exciting opportunity to work with a Not for Profit Organisations providing essential services such as counselling, support groups, InfoLine, Neurological Nurses and education while raising awareness. This role is a unique one to expand your skills across Australia.

The Role:
The Aged Care & Disability – Support Coordinator will coordinate and implement a program in a big range of settings, alongside other members of the Parkinson’s team, to build support to the sustainability of PNSW including NDIS, coordinate case management and individual planning, policy development, research and advocacy. 

As a Support Coordinator, you will do this by establishing a positive collaborative relationship with the participant, their family and/or carer in identifying and connecting with appropriate providers of support. With a strong understanding of Aged Care Services and the NDIS systems, you will ensure the implementation of participants’ plans and achievement of their goals to support each to overcome barriers. This current caseload of clients will be predominantly compost by men over 60 years old of age, affected by mental health.

The Aged Care & Disability – Support Coordinator will be responsible for ensuring the Aged Care & Disability Program plan, deliver care and services in accordance with relevant funded program guidelines, Disability Act 2006, NDIS framework, Aged Care Quality Standards and CORE Policy and Procedures.

The right candidate is also preferably a Former Registered Nurse or a with a similar health professional background.

Key Selection Criteria:
To be considered for this role, you will need to have the following attributes as a minimum:

• BA of Social Work, Social Science, Nursing or equivalent experience 
• A sound understanding of relevant legislation, including NDIS procedural guidelines and the full range of community services that may be required by participants.
• A commitment to work within the NDIS policy and funding framework.
• Relevant tertiary qualifications in health/social sciences as required to provide Support Coordination
• Previous experience working as Support Coordinator
• Drivers Licence and own vehicle
• Willing to undertake NDIS Worker Screening
