About the role

  • Be part of our Complex Disability Support program
  • Work with families providing support & case management in relation to the NDIS
  • Contract until June 30th 2024|full time|working with the leading provider in the state

 

Provide flexible outreach, pool cars available. Conduct assessments and provide case management, helping young people and their families identify needs and meet goals. Based at Bendigo.

 

Your role:

The Complex Disability Family Support role, works alongside families where children with a disability are at risk of requiring care, to provide early help.  The Practitioner will work in partnership with the family in a child-centered, family focused way, providing proactive outreach, with a strong focus on working closely with stakeholders to achieve positive outcomes.

A key focus is to build parental capacity and resilience by delivering a flexible and individually tailored service to families in order to sustain care in the family home.  The Practitioner works collaboratively with key stakeholders to advocate for the disability supports required in the child’s NDIS plan while also building on family strengths, aimed at enhancing parenting capacity, parent-child relationships and social connectedness.  

 

The skills you will use day to day:

  • Demonstrated experience and knowledge of the NDIS and the disability sector, specifically regarding NDIS planning, advocacy and an understanding of the range of services that support children who have a disability and their families. 
  • A strong understanding of issues facing families who have children with a disability and demonstrated experience working with families who have children with a disability, including skills in engagement and holistic assessment of needs and action planning.
  • Demonstrated awareness and commitment to working within the Best Interest Principles outlined in the Children, Youth and Families Act 2005, including demonstrated resilience to work with and support clients who have been exposed to trauma, violence or neglect.
  • Demonstrated ability to work collaboratively with a diverse range of stakeholders to reach the best outcomes for children, young people and families.
