About the role
THE OPPORTUNITY:
We are seeking a dedicated Case Manager to join our team. As a Case Manager, you will play a crucial role in assisting individuals with their housing needs and providing essential support.
RESPONSIBILITIES:
- Maintain a client case load in alignment with program guidelines.
- Utilize a client-centered framework to develop tailored case plans for each individual.
- Support clients in improving their capacity to secure adequate accommodation through holistic case management.
- Proactively assist clients in achieving self-reliance using a strengths-based, solution-focused approach.
- Maintain accurate and professional client records, case notes, and documentation.
KEY SELECTION CRITERIA:
- Formal qualification in Social Work, Community Services, Welfare, Social Sciences, or equivalent (highly desirable).
- Demonstrated experience in a dynamic social service setting with a focus on client responsiveness.
- Experience working with clients with complex needs.
- Familiarity with case management frameworks.
- Understanding of the behaviours exhibited by individuals experiencing homelessness or traumatic circumstances.
- Excellent communication, interpersonal, and negotiation skills, with a sensitivity to cultural awareness and client needs.
