About the opportunity

We have an opportunity for a Case Manager to join our Care and Placement Services Program. The role will deliver case management support to families where children and young people are at imminent risk of entering or returning to residential care. The role will also support children and young people to safely exit residential care and transition to more appropriate care arrangements such as foster care or kinship care.

As a Case Manager, you will undertake comprehensive assessments and provide individualised flexible outreach support to children, young people and families. Working as part of a collaborative care team, the Case Manager will promote placement preservation and aim to prevent placement breakdown.

About you

To be successful in your application you will meet the person specification in the position description, including having a relevant tertiary qualification in social work, welfare work, child development or a related discipline. Candidates who currently hold a Diploma level qualification will be considered if they are willing to complete further studies.

You will also have:

  • Demonstrated experience within the child, youth and family welfare sector
  • An understanding of the Children, Youth and Families Act 2005 and Best Interests Framework
  • Experience in undertaking risk assessments and planning appropriate intervention strategies
  • A strengths-based, child-centered approach to case management
  • Capacity to work flexibly and innovatively to meet the needs of families, children and young people
  • Well-developed communication and organisation skills
