About the role
As an Outreach Case Manager with the Older Persons Support and Connect Program, you’ll provide essential support to older individuals experiencing or at risk of homelessness. This part-time role (24 hours per week) is based in Broadmeadows and focuses on assertive outreach, short-term case management, and linking clients to vital services. You’ll also engage in community development activities to reduce isolation, build connections, and support clients to live independently. We are seeking a candidate with a qualification in community services, social work, or a related field, along with experience working with vulnerable populations, particularly older individuals. The ideal candidate will have strong advocacy and case management skills, a solid understanding of aged care and public housing systems, and a commitment to social equity. Excellent communication, interpersonal, and organisational skills are essential, as is the ability to collaborate effectively and support diverse communities within a team environment.
