Outreach Case Manager – Housing Support for the Aged

As an Outreach Case Manager with the Housing Support for the Aged Program (HSAP), you’ll provide vital support to older individuals experiencing or at risk of homelessness. This part-time role (24 hours per week) is based in Brunswick and focuses on assertive outreach, short-term case management, and connecting clients to essential services. We are seeking a candidate with a qualification in community services, social work, or a related field, along with experience supporting vulnerable populations, especially older adults.
