The Organisation:

By listening and understanding those needs, they ensure a better living experience for every participant and the network of people who support them.

The Role: 

The Intake and Transition Manager plays a critical role in ensuring a seamless and positive experience for individuals transitioning into Adenium Living homes. This position is responsible for the development and management of the intake process, coordinating the participant transition process, and collaborating with various stakeholders to provide comprehensive support to residents and their families. The role requires a combination of organisational skills, interpersonal communication, and a commitment to the well-being of the SDA participants. This is a hands-on, solutions-focused role suited to someone who thrives in a dynamic environment and can confidently balance participant wellbeing with operational delivery.

Reporting to the National GM, Operations & SDA you will: 

Intake Coordination:

  • Take responsibility for day to day support and management of all intake and transition team members and tasks
  • Process new intake and enquiries of SDA services
  • Gather intake information and initial risk identification assessments to determine the needs and preferences of potential SDA participants (inclusive of SDA funding levels)
  • Collaborate with referral sources such as support coordinators, hospitals, and social service agencies
  • Consider participant compatibility matching across new projects/existing projects with vacancies
  • Ensure relevant intake documentation is completed at intake which complies with the NDIS Quality and Safeguarding Practice Standards

Transition Planning:

  • Develop and implement personalised transition plans for residents, considering their unique needs and circumstances
  • Liaise with healthcare professionals, families, and relevant community organisations to coordinate a seamless transition into Adenium Living houses
  • Provide guidance and support to residents and their families throughout the transition process
  • Connect and support participants during the handover process to the tenancy team

The Person: 

You’ll be driven and show genuine passion for building capability within your team. In addition, you: 
  • model a positive attitude, professional conduct and unwavering commitment to quality outcomes and service delivery for staff, individuals with disability and the community 
  • enjoy working with a high performing group of colleagues and peers and accept responsibility and accountability 
  • are an articulate, clear communicator with a high level of literacy 
  • possess a high level of integrity, passion and reliability 
  • In-depth exposure to an established SDA portfolio or community / social housing portfolio 
  • Sound knowledge of NDIS and Specialist Disability Accommodation
  • Customer service and time management skills 
  • Exceptional writing and editing skills with a high level of attention to detail
  • Critical thinker and a problem solver
