About the role

As an Outreach Case Manager in our Hume Program, you’ll walk alongside individuals and families who are homeless or at risk, helping them navigate housing pathways, access vital services, and build stability for the future. Working in a team-based case management model, you won’t be alone—your ideas, experience, and insights will be valued as part of a collaborative and caring team.
You’ll spend much of your time out in the community, meeting people where they are—in their homes, temporary accommodations, or wherever support is needed. Your work will support them to achieve their goals, become more self-reliant, and access the safety and dignity that stable housing can provide.
ABOUT YOU
You’re passionate about social justice and experienced in providing case management or similar support to people experiencing disadvantage. You understand the impact that homelessness has on individuals and families—and you’re committed to doing something about it.
You’ll ideally have:

A relevant tertiary qualification (e.g. social work, community services, youth work), or extensive experience in the field

Strong interpersonal skills with the ability to engage respectfully and effectively with people from diverse backgrounds

Confidence working both independently and in a collaborative team model

Great organisational skills—able to juggle competing priorities and keep accurate case notes
-
Familiarity with the homelessness sector and frameworks like the Opening Doors model is a bonus, but not essential
What matters most is your empathy, resilience, and commitment to supporting people with dignity and respect.
