MS Consultant – Social Worker/Community Support Worker

MS is seeking expressions of interest for a MS Consultant (Social worker or Community Support Worker) for the Southern Tasmanian region. The experienced MS Consultant will engage with people affected by multiple sclerosis in providing person-centred assessment, advice, support, navigation and assistance. Linking customers to internal and external services as required through the application of coordination, negotiation and advocacy skills.

You will be responsible for:

• Working with the customer to explore the circumstances relevant to the persons contact and guide them through information and options to support and address their situation.
• Promoting an integrated, collaborative approach to enable cooperation with the various sectors to support the individual.
• Providing information and advice that will assist the customer to build capabilities, health literacy and empowerment.

We seek an individual with:

• A degree or Diploma in Social Sciences, Health Sciences, or Behavioural Sciences
• Minimum of 2 years’ experience in providing clinical services, support and advice to people with chronic neurological conditions, including MS.
• Sound knowledge of health, community and disability service sector including NDIS.
• Demonstrated ability to network and develop effective working relationships collaborations to achieve targeted interventions, referral and support.
• Demonstrated understanding and an ability to implement the principles of person centred practice

The role is fixed term until 30th June 2017 working up to 30.4 hours per week.
