The primary focus for the group is to connect clients with the right services, at the right time, by the right work force at a cost the community can afford. We provide a range of services including care delivery in the home, consultancy services to facilitate clients remaining at home and actively participating in their local communities with the aim of enabling clients, their families and carer’s to have the highest quality of life achievable for their circumstances.

What are we up to now?
We are establishing an innovative three year pilot with a Metropolitan Melbourne Health Service. Become part of a team that will provide health system navigation coordination to a cohort of patients that have a chronic illness. This primarily community based role will act as the conduit between multiple parts of the health system from a patient perspective and will be a consistent contact point along their journey. The role will also involve a component of direct care clinical assessment and intervention aimed at preventing unplanned or inappropriate admission to hospital.
As we commence the pilot we have multiple positions available to be part of this team.

The fundamentals of a Health System Navigator / Coordinator role

  • Develop client relationships essential to providing the client health system information and chronic condition self-management support and resources.
  • Empower clients and health providers to apply chronic condition self-management principles which will potentially prevent or slow disease progression.
  • Work across interagency, multidisciplinary/interdisciplinary teams.
  • Act as an advocate to the client and their family whilst protecting and upholding their rights.
  • Contribute to and incorporate research findings into Chronic Disease management practices.
  • Be challenged by identifying and maintaining key relationships within both the health service and the community in an environment of competing priorities and demands.


What we’ll require from you

  • Relevant qualification in nursing or allied health with demonstrated experience.
  • Minimum two years post graduate clinical experience in field of expertise.
  • Demonstrated understanding of chronic conditions management.
  • Demonstrated ability to provide education, support and coaching to clients and their carer’s.
