COORDINARE has been established to take up the challenge of delivering the Primary Health Network for South Eastern NSW. COORDINARE utilises a unique blend of private and public perspectives and innovative thinking which aims to transform the health of people in the region. Due to recent changes to the Integrated Team Care program and a new commissioning approach across the region, COORDINARE now has a new role available.
Job Summary:
The Integrated Team Care Coordinator will provide support and leadership to the commissioned Integrated Team Care (ITC) workforce to achieve better treatment and management of chronic conditions for Aboriginal and Torres Strait Islander people, through better access to the required services, better care coordination and provision of supplementary services.
The Role:
COORDINARE is seeking an exceptional health professional to work within the Commissioning team, contributing to improved health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions.
This new position will provide support and leadership to a commissioned Integrated Team Care (ITC) workforce and a range of health care providers in the South Eastern NSW region to achieve better access to services, improved coordination of care and provision of supplementary services for Aboriginal and Torres Strait Islander people.
The Integrated Team Care Coordinator will report to the Aboriginal Health Service Development and Performance Manager.
The Applicant:
We are seeking a person with excellent communication skills, appropriate qualifications and care coordination experience to provide support and leadership to the Integrated Team Care workforce who will be embedded within providers across the South Eastern NSW Primary Health Network catchment.
You will have a comprehensive understanding of Aboriginal and Torres Strait Islander cultures, protocols, issues and affairs as well as expert knowledge of chronic disease management in a primary care setting.
Ideally you will have strong working relationships with Aboriginal health providers and communities in the South Eastern NSW PHN region. Additionally, you will have a current driver’s licence and be willing to travel as part of this role.
