The Inner Melbourne Connections Program (IMCP) is a newly established pilot program, a partnership with Melbourne City Mission. Providing psychosocial and health supports to people who are significantly affected by mental health issues and experiences of homelessness but who are not eligible for support through the NDIS. Providing assertive outreach activities and on site drop in supports, to identify and engage potential clients. Individual case work and planning will then be used to develop packages of care that will identify recovery focused goals, address key health and housing concerns, and identify key barriers clients may be facing in achieving their goals.
About the Role
This newly created position is diverse, where no two days are the same. You will provide a service that aims to enhance the quality of life, build life skills and ensure access to both community mental health and broader health and community services. focusing on client service delivery through facilitative and direct care engagement using CRM, underpinned by the principles of trauma informed care, family centred practice and culturally appropriate practice. Providing a flexible, individualised approach, working in consultation with clients to access needs and develop a range of client directed goals. Care plans and packages offered promote choice, acknowledge individual strengths and facilitate inclusion within the community.
All about you
The ideal candidate will be able to work independently, be an excellent communicator, maintain confidentiality, with self-awareness and the ability to self-regulate during difficult situation, as well as;
- Hold a relevant tertiary qualification or equivalent experience in the housing, mental health, disability or welfare sectors;
- Demonstrated experience in undertaking, supporting and coordinating assertive outreach and group activities;
- A sound understanding of the key issues affecting people with multiple and complex needs including mental illness, homelessness, alcohol and other drug issues, health and safety issues and social connection;
- Understanding of trauma and client-centred models of care; and
- Hold a valid Victorian driver’s licence.
