We are seeking an enthusiastic Case Manager to join our Housing Mental Health Pathways Program (HMHPP) at our Collingwood site. HMHPP seeks to prevent people being discharged from an acute psychiatric inpatient stay into homelessness by providing a specialised housing intervention at the time of a person’s discharge from hospital. HMHPP is a partnership between Launch Housing and St Vincent’s Hospital’s Acute Inpatient Service (AIS) in Fitzroy. Launch Housing has a similar partnership with the Alfred Hospital.
The position involves engaging with people experiencing or at risk of homelessness while they are an inpatient at the psychiatric inpatient unit and working collaboratively with patients, hospital staff and community supports to develop an appropriate housing support plan alongside regular discharge planning processes. In many cases, HMHPP support will continue after a person’s discharge from hospital, until clients have secured appropriate, safe and sustainable housing and connected with any relevant ongoing support/services.
People with multiple and complex needs and limited to no supports are prioritised for referral to HMHPP.
The position operates as a solo worker program. However, the worker sits within the Northern Outreach Team and thus has access to the teams informal and formal support mechanisms including team meetings and specialised external group consultations.
What you will need
The successful candidate will have a tertiary qualification in Social Work, Nursing, Psychology, Occupational Therapy or other relevant allied health discipline. They will possess a thorough understanding the mental health and housing/homelessness systems, policy and context coupled with significant practical experience working collaboratively with people who experience complex mental health issues and housing instability towards short to long term goals.
They will also possess excellent interpersonal skills, take accountability for their work outcomes and be able to form and maintain strong relationships – all essential ingredients to working collaboratively with consumers and other organisations to achieve common goals.
The successful candidate will also have outstanding time management skills with the ability to self-direct and work independently when needed. An understanding of the time pressures and changing priorities of hospital discharge planning will be highly regarded.
