About the opportunity
The Home Care Package Case Manager works in partnership with consumers, carers, families and informal supporters to facilitate the consumer directed support and services that may be required to meet the client’s goals; assistance to help older people remain living in their home in line with Uniting principles and funding guidelines.
You will have experience and a well-developed knowledge working in the Aged & Carer services sector and in a case management role. You will have a associate diploma in Social Work or Health, Welfare discipline. Outstanding customer service, quality of care and continuous quality improvement are the hallmarks of this position.
