We are a secular, independent provider of choice for clients and the ‘go to’ organisation on homelessness for government, media, philanthropy, private donors and the community.

In August 2018, as part of Victoria’s Homelessness and Rough Sleeping Action Plan, the Department of Health and Human Services (DHHS) announced a range of new rough sleeper programs across metropolitan Melbourne as well as a number of regional centres across Victoria. As a result, the new position of Intensive Case Manager has been created.

 

About the role

The Intensive Case Manager is a part of the Supportive Housing team who provides intensive, flexible, and responsive client focused support with the aim of assisting clients to successfully obtain & sustain housing, improve physical and mental health and develop connections within the local community.

This role is responsible for providing assertive and persistent outreach to people who are sleeping rough to progress goal-oriented intensive case management for clients with complex needs and challenging behaviors. To accomplish this, the Intensive Case Manager will develop case plans in conjunction with clients and implement strategies that enable clients to achieve their goals by building on their strengths and focusing on solutions.​

 

What you will need

You will be tertiary qualified/ have direct experience and have a comprehensive understanding of the issues which those experiencing homelessness confront and possess the skills and aptitude in working for people who are marginalized and disenfranchised with complex needs.
