About the Role

This is a great opportunity for a Support Coordinator to partner with our clients and or their representative in the ongoing coordination of their needs, goals, aspirations, choices, expectations, motivations, preferences, values and financial profile.  By maintaining a high touch relationship you are responsive to changes in circumstances and respond accordingly.   Review and amend support plans, budgets and fee schedules in response to changing client circumstance as part of the agreed service provided by Jewish Care Victoria.

 

Skills and Experience

  • Tertiary Qualifications in Human Services, Nursing, Allied Health, Health Management or similar
  • Experience in community aged care re-ablement programs
  • Delivery of programs in a community setting
  • Demonstrates a comprehensive understanding of the target population and their needs, across the continuum from simple support advice to complex care coordination
  • Experience in reviewing / amending client support plans, budgets and fee schedules in response to changes in the client circumstance
  • Able to work closely with the Customer Contact Team, the Support Advisor Team and the Support Services Team to ensure services and products are delivered as negotiated.
  • Up to date knowledge of assistive and other technologies that support ageing.
