Who we’re looking for
We have a fantastic opportunity to join our growing NDIS Support Coordination team.

If you’re a Support Coordinator, qualified in Mental Health, AOD, Disability or a health or human service-related discipline, enjoy variety in your day, and working closely with other service delivery professionals, you might be exactly the person we’re looking for!

Support Coordinators play a critical role in supporting participants to navigate the NDIS system, unde rstand their NDIS plans, connect into the broader system of services and to build their capacity to direct their supports independently over time. As part of our Coordination team at Ringwood, EACH is looking for 2, full-time Support Coordinators to provide services, making a huge difference to the lives of those living in the community with a focus on NDIS connections.

This position would suit someone with a strong understanding of the NDIS and an ability to support participants into appropriate supports and services to achieve the identified goals of their plan.

You will have demonstrated experience in community participation and service coordination that has improved the lives of people with a disability along with extensive and successful experience in managing a range of stakeholder relationships and multidisciplinary teamwork. Your experience in clinical, educational or community-based disability, mental health or early childhood intervention services or related fields will be highly regarded.

An ability to work creatively and collaboratively to solve problems and a strong understanding of managing services within a funded plan and budget and a commitment to professional and ethical conduct and the values of the organisation are also required.

What you’ll be doing
As a Support Coordinator, you will support NDIS participants to build capacity to coordinate their NDIS funded plans, navigate, negotiate appropriate support and services, and connect with community support and mainstream services. You will provide connection and coordination and support participants to both establish new services under NDIS and transition from old services to NDIS.

Knowledge of the NDIS system and processes, excellent verbal and written communication skills, teamwork, high-level organisational skills and a demonstrated ability to utilise person-centred practices will assist you in you r workday. Your day will include actively managing a client caseload, attending meetings (office-based, community-based, and home visits), administrative tasks associated with coordinating services and report writing.

Who you’ll be working with
Coordinators will work inclusively with participants, families, and carers to build great relationships and enhance participant supports. This position is part of a team of currently 4 Support Coordinators who share information and skills, reporting to an onsite, engaged Team Leader.
