Our Health and Wellbeing Unit is seeking an NDIS Support Coordinator (0.8 – 1.0 EFT) to join the Care Coordination team at Sunbury Community Health. The primary function of this position is to assist National Disability Insurance Scheme (NDIS) participants to activate their NDIS plan. This includes assisting participants to understand their plan, understand NDIS processes, identify service options, and manage their allocated individual budget.
Applicants with a tertiary qualification in Social Work or Diploma in Community Services or related diploma and knowledge and experience in the disability sector are encouraged to apply.
