About the role

About the Opportunity

Northside now has an exciting opportunity for a Relationship Coordinator to join their team in Canberra, ACT on a full time basis.

Reporting to the Clinical Care Manager, you will primarily be responsible for client engagement and entry into the Aged Care and Inclusion programs, with a particular focus on the Commonwealth Home Support (CHSP), Home Care Package (HCP) and Veterans’ Home Care (VHC) programs.

Your key responsibilities will include, but will not be limited to:

  • Ensuring a seamless, respectful and positive experience for all individuals from initial enquiry through to commencement of services;
  • Providing case coordination support to connect clients to supports;
  • Developing and maintaining strong working relationships with referral agencies and other relevant stakeholders;
  • Increasing the current client base through promoting and marketing services; and
  • Developing and implementing processes for the intake, assessment and onboarding of new clients in accordance with Client Directed Care (CDC) principles.

About You

To be considered for this role, you must hold a Certificate IV, preferably in an Aged Care or Community related discipline, or be willing to work towards this. Northside are willing to enrol and support the successful candidate in completing your qualification.

You will need to have demonstrated administrative experience with a high level of computer literacy and strong skills in database management and Microsoft Office as a minimum.

You should have an understanding of Aged and/or Disability sector reforms, as well as the impacts of portable funding on service delivery and client experience. You will also need a sound understanding on the Community sector, including knowledge of the Aged & Disability, LGBTIQ, Aboriginal & Torres Strait Islander and CALD communities.

In this fast-paced position and environment, you will need to be flexible and adaptable to change. Your strong time management, problem solving and organisational skills will be essential to your success in this role.

Importantly, you must have exceptional communication skills as well as the ability to negotiate and resolve conflict when necessary. You will need to be able to establish and maintain positive relationships with a number of stakeholders, so your interpersonal skills will be key.

Finally, you’ll be passionate about supporting marginalised or vulnerable individuals and delivering services that will make a real in the community. You’ll understand the importance of social participation and community connectedness, with a real interest in the work that Northside delivers.

Please note: You will need a current ‘C’ Class Driver’s License and access to a reliable vehicle. You will need to also hold, or be willing to obtain, a First Aid Certificate, Working with Vulnerable People Card and complete a Police Check.
