About the role

  • Fixed-term (through to June 2021), Part-time role (60.8hours per fortnight)
  • Salary: $73,083 – $78,637 per annum (pro-rata) + Super
  • Great Cheltenham location, working from home initially due to COVID-19 measures

About the role:

Eastern Region Mental Health Association (ermha) and Launch Housing have formed a partnership to deliver the National Psychosocial Support and Continuation of Support programs (NPS/COS) to consumers and their families / carers across south east Melbourne.

NPS/COS programs are federally funded via South East Melbourne Primary Health Network (SEMPHN) program and aim to assists people with severe and persistent mental illness who have complex needs which are not currently being met by the existing service system and/or are waiting to be tested and gain access to the NDIS.

This role provides assertive outreach and care-coordination to support individuals to develop skills and abilities to manage their day to day lives. These individuals have often experienced trauma, have multiple complex needs and have been diagnosed with a mental disorder. The goal is to enable rather than do and promote independence, development and confidence of the individuals by designing and developing a case plan and working with relevant services.

This role also involves assertive outreach and facilitating groups with cohorts that experience additional barriers to accessing psychosocial support.

About you:

To be successful in this role, you must have a tertiary qualification in social work, psychology, mental health, Alcohol and Other Drugs or a related allied health profession, or equivalent experience. You will also have experience working with people who have multiple, complex needs and/or have been diagnosed with a mental disorder. You are passionate about providing recovery-focused, person-centred support to people.
